Saturday, April 12 from 10am to 1pm,

Saturday, May 10th from 10am to 1pm and Saturday,

June 14 from 10am to 1pm.

What You Need to Apply for a Passport:

  1. Completed Passport Applicaton Form DS – 11

      Complete the Form using full names (no initials or abbreviations).

      Do not sign the application until requested to do so.

  1. Proof of US Citizenship

Please Note: Hospital and Church Certificates are NOT acceptable. An expired passport is acceptable proof of citizenship; however, children under 14 who have had a previous passport are required to have the expired passport, along with a Birth Certificate.

  1. Proof of Identity

4. Two (2) professionally developed passport photos. 

5. Payment

   $67 for adults 16 and older (includes a $12 security fee).

$52 for anyone under 16 (includes a $12 security fee). (The U.S. Department of State only accepts checks or money orders).

*For Expedited Service, add $60 to the U.S. Department of State fee. This Expedited Service fee does not include overnight delivery. You must submit overnight delivery forms, such as Express Mail, and postage to ensure receipt within 2 weeks. 

  1. To Check Status of Your Passport - You can check on-line at http://travel.state.gov/passport/get/status/status2567.html Enter your last name, date of birth, and the last 4 digits of your social security number. Or call toll free (877) 487-2778. TDD for hearing impaired: 1-888-874-7793.

NEED A PASSPORT QUICKLY? USE EXPRESS MAIL SERVICE TO AND FROM THE NATIONAL PASSPORT AGENCY. PREPAID, ADDRESSED ENVELOPES ARE AVAILABLE AT YOUR LOCAL POST OFFICE FOR YOUR CONVENIENCE.
